Basketball Waiver of Liability 

1. Acknowledgment and Assumption of Risk

As the parent and/or legal guardian, I give permission for my child to participate in basketball and related athletic activities organized by PNW Rain LLC and held at Valley Dreams Sports Hub, LLC or other designated facilities. I understand that participation involves physical activity and may expose participants to certain risks, including but not limited to slips, falls, collisions, equipment failure, and other unforeseeable hazards that can result in minor or serious injury, permanent disability, or death.

I knowingly and freely assume all such risks, both known and unknown, even those arising from the negligence of the “Releasees” (as defined below), and I accept full responsibility for my child’s participation.

2. Release and Waiver

In consideration of being permitted to participate in any PNW Rain LLC or Valley Dreams Sports Hub, LLC activities, I, for myself, my child, and our respective heirs, assigns, and representatives, hereby release, waive, discharge, and hold harmless:

PNW Rain LLC, Valley Dreams Sports Hub, LLC their respective owners, directors, officers, employees, agents, volunteers, contractors, sponsors, advertisers, affiliated organizations, and any facility owners or lessors (collectively, the “Releasees”) —from any and all liability, claims, demands, losses, or damages on account of injury, illness, disability, death, or property damage that may occur as a result of participation in these activities, whether caused by the negligence of the Releasees or otherwise.

3. Medical Consent

I authorize PNW Rain LLC and Valley Dreams Sports Hub, LLC staff or representatives to obtain emergency medical treatment for my child in the event of injury or illness while participating in any activity if I cannot be reached. I accept full responsibility for any resulting medical expenses.

4. Media Release

I acknowledge and agree that photographs, videos, and other media taken during activities may be used by PNW Rain LLC and Valley Dreams Sports Hub, LLC for marketing, promotional, or educational purposes in print or digital media. I grant full permission for the use of my child’s image without compensation and understand these materials become the property of PNW Rain LLC.

5. Code of Conduct

I understand that participants are expected to demonstrate respect toward coaches, teammates, opponents, officials, and facilities. PNW Rain LLC and Valley Sports Hub, LLC reserves the right to dismiss any participant whose behavior is deemed unsafe, disrespectful, or detrimental to the program without refund.

6. Agreement and Signature

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. I SIGN IT FREELY AND VOLUNTARILY, WITHOUT ANY INDUCEMENT, ON BEHALF OF MYSELF AND THE PARTICIPANT.
